A lead qualification survey should identify fit, need and readiness, then send each respondent to an appropriate next step. It is a short decision system: every question must map to an explicit score, segment, routing rule or necessary analysis field.
What is a lead qualification survey?
A lead qualification survey is an interactive questionnaire that uses declared answers to sort a contact into defined segments. A useful outcome may be a sales handoff, educational sequence, self-serve recommendation or respectful no-fit result. The survey should collect only information that changes that outcome.
What should a qualification survey measure?

Budget is not a universal first question. Ask it only when price fit actually changes the route; otherwise it can confuse incomplete problem understanding with inability to pay.

- How often does the problem occur?
- Which capability is mandatory?
- How many people or records would the workflow support?
- Which systems must it connect with?
- What role do you have in evaluating a solution?

- What would a useful next step be?
Every answer option needs an action. If an option never affects scoring, segmentation, personalization or analysis, remove the question or document why it is collected.

Starting hypotheses: 75–100 sales-ready, 45–74 nurture, and 0–44 self-serve or no-fit. These thresholds are examples, not benchmarks. Validate them against accepted opportunities, closed outcomes and false positives before operational use. Worked example: let a hard constraint override the score
A respondent scores 80 but selects a mandatory integration the product does not support. The hard constraint should override the average. Route the person to an honest no-fit result or alternative workflow and preserve the reason. A weighted score must not average away a disqualifier.
Native versus integrated follow-up

How do you validate the routing before launch?
- Create one synthetic respondent for every terminal segment, hard constraint and boundary score.
- Calculate the score by hand and compare it with the platform result.
- Confirm the source answers, score, segment and routing reason reach the downstream record together.
- Deliberately interrupt an integration and verify that the failure is visible and recoverable.
- Review early accepted and rejected outcomes for false positives and false negatives, then version any rule change.
Limitations
Self-reported intent can change, and scoring models reproduce the assumptions built into them. Do not infer sensitive traits, hide consequential automation or treat a score as objective truth. The worked weights and thresholds above are a reproducible starting template, not measured conversion benchmarks.
